Retiree Forms

Frequently requested forms are listed below. Each form opens in a PDF format, which is good for viewing and printing. You must have Adobe Reader installed on your computer. Some of these forms are also available if you log in to My Account.

Name or Address Change. If your name or address changes, you can let us know of the change by logging in to My Account. If you prefer, print the PDF of this form and mail it in. You must notify IPERS of an address change even if you already notified the post office. It’s important to keep your address up to date so IPERS can send important information to you. online | pdf
Application for Service Purchase. Use this form if you are interested in receiving a price quote to purchase service as a way to increase your benefit. Read more about how a service purchase works by selecting Benefit Information from the left-hand menu. online | pdf
Direct Deposit Authorization. Use this form to have IPERS directly deposit your monthly benefit payments in your bank account. It is faster and safer than sending a check by mail—and it’s free. (There is an administrative fee for paper checks.) online | pdf
Reemployment Termination Verification. Use this form if you retired, returned to work, and retired a second time. You are responsible for completing this form whenever you leave your reemployment. online | pdf
W-4P Withholding. Complete this form to adjust the amount withheld from your pension payment. online | pdf
Model QDRO. A Qualified Domestic Relations Order (QDRO) is a special court order that specifies how pension assets and property will be divided in a divorce. This model QDRO can be used as a template by lawyers and the court to create a QDRO. Authorization for Release of Information. This form authorizes IPERS to release and discuss specified information from a member's IPERS account with another named party.
