Changes to Notarization Requirements

Until further notice, IPERS no longer requires notarization or witnessing of a member’s signature on some applications as shown below.  

Application for Retirement (Including Actuarial Equivalents)

If you are not married: IPERS no longer requires notarization or witnessing of a member's signature if the member provides a legible copy of appropriate identification.

If you are married: IPERS no longer requires notarization or witnessing of the member's signature or the member's spouse's signature if the spouse is the beneficiary of the death benefit or is the contingent annuitant. The member and spouse must submit a legible copy of appropriate identification.

Signatures of an alternate payee or successor alternate payee: IPERS no longer requires notarization or witnessing if a legible copy of appropriate identification is provided. 

Application for Death Benefits

IPERS no longer requires notarization or witnessing of the applicant's signature if the applicant provides a legible copy of appropriate identification. 

Application for Retired Reemployed Benefits

IPERS no longer requires notarization or witnessing of a member’s signature if the member provides a legible copy of appropriate identification.

Appropriate identification: An Iowa driver's license or identification card, another state's real ID, or a passport
